3. Scanned copy of the Signed Performance Rating in the last rating period (if applicable).

4. Scanned copy/ies of Authenticated Certificate of Eligibility/board rating/valid license.

5. Scanned copy of Transcript of Records.

6. Scanned copy of training/seminar Certificates (relevant only to the position).

APPLICATIONS WITH INCOMPLETE DOCUMENTS SHALL NOT BE ACCEPTED.

Important Reminders:

1. An incomplete and unclear copy of the Personal Data Sheet shall NOT be processed. It is essential for these documents to be clear for the initial evaluation of applications.

2. Only applications submitted on or before the deadline will be considered.

3. Only shortlisted applicants shall be notified;

4. The dates in the Personal Data Sheet, Work Experience Sheet, and Notarization shall be within the period of publication only.
